What to bring for your first appointment
To ensure your first appointment runs smoothly, please bring the following:
For All Patients
- List of current medications: This includes prescription medications, over-the-counter medications, and any supplements you are taking.
- Relevant medical history: Please bring any information about previous injuries, surgeries, or other health conditions that may be relevant to your current condition. This might include referral letters from your doctor, or reports from other healthcare professionals.
- Comfortable clothing: Wear loose, comfortable clothing that will allow the physiotherapist to easily assess and treat your injury. For example, shorts for leg injuries, or a tank top for shoulder injuries.
- Health insurance information: Please bring your health insurance card so we can process your claim.
For Patients with a Medicare EPC Referral
If your doctor has provided you with an Enhanced Primary Care (EPC) referral (formerly known as a Chronic Disease Management (CDM) plan), please bring the following:
- Your EPC referral form: This is essential for us to process your Medicare rebate.
- Your Medicare card: We will need this to process your rebate.
All Medicare (EPC) New Patient Appointments must be made over the phone NO ONLINE BOOKINGS PLEASE.
For Compensable Patients (WorkCover, TAC, DVA, NDIS)
In addition to the general information above, if your treatment is covered by a compensation scheme (such as WorkCover, TAC, or DVA), please also bring:
- Claim or client number: This is essential for us to process your claim.
- Case manager details (if applicable): Including their name, phone number, and email address.
- Plan Management Company (for NDIS)
- Employer details (for WorkCover): Including the name and contact information of your employer.
- Referral letter from your doctor (if required): Some compensation schemes require a referral letter before treatment can commence.
All Compensable New Patient Appointments must be made over the phone NO ONLINE BOOKINGS PLEASE.
Important Information Regarding Fees
Please note that you will be expected to pay for your service on the day of your appointment. We accept cash, EFTPOS, credit card.
- For all patients (excluding Compensable Patients): We will process your health insurance claim for you where applicable, and advise you of the expected gap payment (if any) on payment.
- For Compensable patients: Your first appointment must be paid for upfront by you on the day of your appointment. You may then claim it back through NDIS/TAC/WorkCover or your employer. We will liaise with your insurer regarding payment for your ongoing treatment, but any gap payments or co-payments will be your responsibility on the day.
- For patients with an EPC referral: You will need to pay for your consultation on the day of your appointment. We will then process your Medicare rebate, which will be refunded directly to your nominated bank account associated with your Medicare account. Please be aware that there may be a gap payment for EPC consultations.
Cancellation Policy
Cancellations with less than 24 hours notice from your scheduled appointment will incur a fee.
